ENROLLMENT PROCESS

• Forms are fax or submitted Online to IBS
• IBS acknowledges receipt within 24 hrs
• Forms are reviewed for accuracy, entered in to HR system
• IBS calls employee to review plan options answer questions
• Forms are submitted to company within 24 hours of receipt
• IBS calls to confirm receipt of forms and ensure acceptance
• Member acceptance is emailed to company contact
• Member ID and plan information updated in HR System
